Release Form

If you are a South Windsor High graduate and would like to request a transcript, please download the release form (above) and send it to the School Counseling Department. You may also submit your request by Fax.

Please provide the following information:

• Your full name at the time of graduation
• Your date of birth
• Year of Graduation
• Where you would like the transcript to be mailed
• Your telephone number in case we need to reach you
• Your signature

Please note that an official transcript must be sent directly from South Windsor High School to your college or institution. Copies that are sent directly to you are considered Unofficial.

Please allow up to two weeks for processing.
