Physical exams are required at three times during a student's years in South Windsor Public Schools. As indicated below, updated Health Assessment Records are required to be completed and submitted to the school nurse’s office by no later than the first day of school in the year noted:
- Students entering Kindergarten: We will accept a Health Assessment Record for a physical completed within one year prior to the start of kindergarten.
- Prior to entering 7th grade: We will accept a Health Assessment Record for a physical completed at any time between the first day of a student's 5th grade year and the first day of the student's 7th grade year. Any student who is not in compliance with this state mandate by the first day of 7th grade will be sent home.
- Prior to entering 11th grade: We will accept a Health Assessment Record for a physical completed at any time between the first day of a student's 9th grade year and the first day of the student's 11th grade year. Any student who is not in compliance with this state mandate by the first day of 11th grade will be sent home.
Special Circumstances for New Students:
Transfers from Within Connecticut. If you are transferring to South Windsor from another school within Connecticut, we will accept a copy of the student's most recent physical examination.
Transfers from Outside of Connecticut. If you are coming to South Windsor from a different state or country, we will accept a copy of a physical exam dated within one (1) year prior to entry into the South Windsor school district. The exam must be completed by a U.S. physician.
In both cases, the Health Assessment Record should be provided at the time of registration, but in no event may it be later than the student's first day of attendance in school. (The updated physical requirements noted above still apply.) -
